Questions I would add:
What is the objective?
What method(s) of instruction is the teacher using to help students achieve the objective?
On a scale of 1-10, what is the overall level of student engagement? (1 being poor, 10 being excellent)
How would you describe the classroom's physical environment (learning centers, organization, seating, student work posted, other evidence of teaching and learning)?
Any other additions to the scale of 1-10 items?
I would caution the use of words such as "correctly" and "properly" unless they are qualified with specifics. Your thoughts?

Is the teacher skillfully guiding direct instruction?
Is cooperative learning taking place?
What instructional strategies are being used?
Is technology being used and how?
What type of evidence do we see to show that students are learning?
These are some thoughts. You do not have to use all of them.

We had a lot of commonalities in our ideas. Thanks for all the input! In my experience, you only have so much time to get your snapshot, so I tried to keep this short and have condensed and broadened the main concepts as well as removed questions or checklist items that refer to specific school programming (such as DI, etc.). Questions are grouped together. Colors turned out funny, so don't mind that, please. :-) Note any changes on the list of questions below before 9pm Saturday, and I'll post the final once I get the little ones to bed. Thanks!
